Endovascular Treatment of Scalp AVF Following Hair Transplantation: Case Report

Scalp arteriovenous fistulas (AVF) following hair transplantation are rare; few single case reports have been described. Therapeutic options include open surgery, percutaneous, and transarterial endovascular embolization; in recent years, these latter gained favour because of their minimally invasive approach also concerning post-treatment cutaneous appearance. A 29-year-old male after 3 months from a procedure began to notice little pulsatile mass beneath the skin forehead. Subsequent diagnostic imaging (CTA angiography) showed presence scalp frontoparietal AVF with superficial temporal artery (STA) feeder subcutaneous venous drainage. After months, sudden worsening symptoms was reported. Endovascular treatment performed general anaesthesia, right femoral arterial access, ECA catheterization, superselective STA microcatheterization at fistula point, embolization platinum detachable coils. No procedural complications occurred. Patient discharged on 4th day, asymptomatic. Clinical resolved promptly just 3–4 weeks. control angiogram 6 complete disappearance fistula. Hair procedures associated low risk profile, is very rare complication; both interventions surgery safe effective treating this complication. preliminary angiographic study needed; if vascular anatomy favourable, can be performed. Coiling occlusion compared glue avoids discoloration and/or among fistulous point. In case, technique proven effective.
